In addition to
injury management, we are pleased to offer the following services:

assessment and programs designed specifically to manage physical challenges
Fee Schedule
Our physiotherapists
limit visits to 1 or 2 clients per hour. Thirty and sixty minute appointments
are available. Accordingly, our fee schedule is based on the one
on one time that you will spend in consultation/treatment with your therapist.
To book a 60 minute appointment please book 2 consecutive 30 minute appointments.
In addition
to your scheduled appointment time, FORM’s exercise space may be
suggested as part of your rehabilitation.
Please enquire
about government subsidies. We are knowledgeable about what may be available
to you.
Form
Physiotherapy's fees are $65 for 30 minutes and $130 for 60 minutes,
payable by cash or cheque. Receipts suitable for submission to extended
health care benefit plans will be issued.
Payment Options/Insurance
Claims
- We accept
cash or a personal cheque made out to your physiotherapist.
- We are not
currently offering physiotherapy services for WCB-related injuries.
- ICBC Claims
will be dealt with on a claim-by-claim basis.
Co-owners
Nadine Plotnikoff (M.Sc.P.T.) and Wendy Epp (B.Sc. P.T.)
offer a combined 28 years of physiotherapy practise in the Lower Mainland.
They have worked as physiotherapists for several of Canada’s National
teams including snowboarding, wrestling, and freestyle skiing in addition
to being involved with professional and amateur athletes from such sports
as triathlon, hockey and football.
Epp and Plotnikoff work in a unique one on one environment,
offering extended visits of 30 and/or 60 minutes duration where diagnostic
assessment, a clear treatment plan and excellent communication with your
medical team are the key objectives. This working environment allows for
thorough evaluation of posture, muscle balance and joint function, and
provides time for education and instruction on home exercises which increases
the likelihood for a successful recovery.Epp and Plotnikoff are also active
members and contributors with the Pacific Sport National Training Centre
and the Sport Medicine Council of BC. They have cultured long-standing
working relationships with Lower Mainland surgeons, general practitioners
and sport medicine specialists. Wendy is an active speaker on Hurt vs
Harm, Running Injury Prevention, and Elite Athlete Functional Testing.
Nadine’s completion of a Masters level thesis has enhanced her skills
specific to management of shoulder injuries. In addition to her experience
in working with elite swimmers and throwers, she has an interest in the
rehabilitation needs of the early osteoarthritic population.
