Added December 8, 2025: In accordance with guidelines recently released by our Physiotherapy Association, we will be increasing our fees, effective January 1, 2026 by approximately 5%. ICBC and MSP fees will be exempt from this increase.

We thank you for your ongoing trust in our delivery of care. Please read below for specific rates.

Please note that we currently accept cash, debit, Visa, MasterCard & ApplePay.

Form Physiotherapy does not do direct billing to extended health providers. We will print you a detailed receipt to apply to your extended health plan and receive a reimbursement.

  • Private physiotherapy sessions do not require a physicians referral. We encourage you to check your extended health coverage as some insurance companies will require a doctors note for fee reimbursement.

    • 45 Minute Initial Assessment: $171

    • 30 Minute Follow Up Session: $114

    • 45 Minute Follow Up Session: $171

    • 60 Minute Follow Up Session: $228

  • Shockwave therapy sessions do not require a physician’s referral, though an initial assessment is recommended for new clients to determine whether Shockwave is the right treatment for you. Please check with your extended health to see if this treatment is covered.

    45 Minute Initial consult + Shockwave (new clients): $191

    30 Minute Existing patient + Shockwave: $134

    15 Minute Follow Up Shockwave: $77

    60 Minute Physiotherapy + Shockwave: $248

    Note: Shockwave Therapy appointments include an additional $20 charge to the normal rate.

    • 45 Minute Initial Assessment: $169

    • Group Exercises Classes: $75 x 10 Exercise Sessions, $150 x 2 Education and Exercise Sessions = $1050 Total

  • ICBC Patients are not required to have a doctors referral to attend physiotherapy sessions. Please attend your session with your claim number and adjuster information.

    ICBC Coverage is as follows:

    • ICBC Physiotherapy Coverage: ICBC patients are pre-approved for 25 visits within the first 12-week period from the date of their accident

    • ICBC Active Rehab Coverage: ICBC patients are pre-approved for 12 visits within the first 12-week period from the date of their accident

    *Patients can receive their Physiotherapy Sessions concurrent to their Active Rehab Sessions.

    At Form Physiotherapy, ICBC Patients are subject to a User Fee for Physiotherapy Services and Active Rehab Services. User Fees are as follows:

    • 45 Minute Initial Assessment: $26

    • 30 Minute Follow Up Session: $26

    • 45 Minute Follow Up Session: $77

    • 60 Minute Follow Up Session: $128

  • The government copay is available on a limited basis to BC Residents on income assistance. We will be able to verify if you qualify for this at your first visit.

    • 45 Minute Initial Assessment: $130 (with eligibility)

    • 30 Minute Follow Up Session: $72 (with eligibility)

  • Telehealth session can be done through video or telephone communication. If you are booking your appointment online, please specify in the notes section which communication you prefer.

    • 45 Minute Initial Assessment: $171

    • 30 Minute Follow Up Session: $114

    • 45 Minute Follow Up Session: $171

    • 60 Minute Follow Up Session: $228
